DESCRIPTION OF SERVICES. The contractor shall provide all management, tools, labor, transportation, sample containers, sample labels, chain-of-custody forms, preservatives, transport containers, and any other equipment necessary to perform laboratory analysis of asbestos and asbestos containing materials. 2. 0. TESTING AND ANALYSIS. 2.1. The contractor shall be available to the government representative, within two hours of notification, to accept samples for testing and analysis. Samples shall be delivered and accepted between 7:30 A.M. and 4:30 P.M. Monday through Friday, excluding federal holidays. 2.1.1. The contractor shall maintain appropriate chain-of-custody, including associated paperwork, at all times. 2.1.2. Sampling will include, but is not limited to the following media: 25 MM polycarbonate cassettes, floor tile, thermal insulation, roofing, mastics, concrete, wire insulation, sheet rock, and drywall mud. 3.0. TESTING METHODS: 3.1 Polarized Light Microscopy (PLM); PLM EPA method 600/R-93/116. Up to 3 layers must be billed as one layer, with reports on all 3 layers individually, along with customer sample number. Additional layers become a separate charge and reported with the other layers. 3.1.1. Phase Contrast Microscopy (PCM) 3.1.2. Any method may be requested for questioned samples or for more in-depth analysis of samples. Approved EPA, NVLAB, and NIOSH-accredited methods and labs shall be used. 4. 0. ANNUAL WORKLOAD ESTIMATES. 4.1. Bulk samples (PLM): Not To Exceed 500 Air Samples (PCM): Not To Exceed 160 5. 0. CONTRACTOR PROVIDED ITEMS AND SERVICES. The contractor shall provide the following items and services to the government, to the parties/locations indicated: 5.1. Within 5 business days of contract award: 5.1.1. One copy of the contractor's written quality assurance program. 5.1.2. Appropriate documentation identifying the specific analytical procedure used to perform the required analysis. This documentation must include all steps used for the analysis. 5.1.3. New, replacement mixed cellulose ester (MCE) filters for PCM analysis. 5.2. Asbestos Testing and Analysis. Within 8 hours of acceptance of rush samples or within 48 hours of acceptance of routine samples by the contractor or his representative, the contractor shall provide: 5.2.1. Written results of the testing and analysis. Facsimiles are acceptable. 5.2.2. Both air and bulk sample reports shall be provided to: 354 CES/CEOMI, 2310 Central Ave, Eielson AFB AK 99702, fax 907-377-1414. 5.2.3. Air sample reports shall also be provided to: 354 MDOS/SGOAB, 2630 Central Ave, Eielson AFB AK, 99702, fax 907-377-2233. 6.0. GOVERNMENT PROVIDED ITEMS AND SERVICES. The government shall provide the labor and equipment needed to pick-up the sampling equipment (sample containers, sample labels, sample transport containers, chain-of-custody forms) provided by the contractor, obtain and document the samples, and deliver the samples to the contractor.
